Donna M. Carollo, LMFT

Marriage Counseling in South Orange County, San Juan Capistrano, CA, provided by Donna Carollo. Helping children, couples, and individuals through psychotherapy. Find healing through art and recovery classes.

Who do you see in your practice?

I provide counseling to individuals, couples, families and children. We address the thoughts and behaviors that you want to change through art therapy, talk therapy and play therapy. work with a wide range of issues from crisis, addiction, marital conflict, and self esteem struggles.

What are your rates?

Currently my standard fee for a 50-minute session is $90 for individuals and $120 for couples and families. Payment is due at the time of treatment unless otherwise arranged.  I accept cash, check, Visa, MasterCard, and American Express.  For self-pay clients,  I can provide a "super bill" at the end of the month that documents the charges, payments, and diagnosis when necessary for insurance re-imbursement.

Do you have a sliding scale?

During these difficult times of job layoffs and financial stress, a time when people most need someone to talk to, I offer a sliding scale based on your ability to pay if you are jobless or a student.  I also will consider the possibility of meeting less often than once a week if necessary.

What is your cancellation policy?

If you do not appear for your scheduled appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the treatment unless we are able to reschedule in that same week.

Do you take insurance?

I currently accept TRICARE and MHN insurance.

Often, my services are covered by mental health PPO insurance policies, but I do not personally submit the paperwork to the company. I will provide you with a super bill at the end of the month, which you can submit to your provider. The insurance company then issues a reimbursement check to you. It is up to your insurance company as to whether they pay for services provided by psychotherapy.  Patients are responsible for payment at the time of treatment.

It is your responsibility to know the type of coverage you carry and the limits of that coverage. Often companies offer a higher percentage of fee coverage for those therapists enrolled on their panel. They pay a lesser amount or percentage to therapists not enrolled on their panel. I have intentionally chosen not to become a part of insurance panels because I do not want these companies to dictate the kind of therapy I provide or to set limits on the amount of time I can work with a given patient.

To determine if you have mental health coverage, the first thing you should do is check with your insurance carrier. Check your coverage carefully and find the answers to the following questions:

  • What are my mental health benefits? And what is the coverage amount per therapy session?
  • How many therapy sessions does my plan cover?
  • How much does my insurance pay for an out-of-network provider?
  • Is approval required from my primary care physician?
